Nursing Pin Order Form

Please complete the West Virginia University pin order form and return to Lorie Stowers, WVU School of Nursing, Office of Student & Alumni Services, 6300 HSC So., PO Box 9600, Morgantown, WV 26505-9600 no later than February 28, 2020.

Graduation Application

All Undergraduate Nursing Students must apply for graduation through DegreeWorks by March 6, 2020:

  1. Log in to Portal (https://portal.wvu.edu)
  2. Click on the DegreeWorks Tab
  3. Next to the Graduation Application field, click the "Apply to Graduate" link

Resume

BSN and BS/BA-BSN students are to submit their Resume. (RN-BSN students do not need to turn in their Resume.) Please send your Resume to Lorie Stowers at lmstowers@hsc.wvu.edu

Community Service Requirements

Community Service Hours must be submitted no later than April 24, 2020.

  • BSN - 100 Community Service Hours Required
  • BS/BA-BSN – 50 Community Service Hours Required
  • RN-BSN – 25 Community Service Hours Required

Licensure and Transcripts

Important: The information listed here is general information and mostly pertains to the WV State Board of Nursing application process and requirements. Each state board of nursing has different application requirements and procedures. Please check the NCSBN website to find the requirements for the state in which you plan to apply for licensure.

Student Instructions about Licensure May 2020

    • The Degree Conferral date is May 17, 2020.
    • The NCLEX-RN Program Codes for the WVU School of Nursing are as follows:
      • BSN (4-year, Morgantown Campus): US30508000
      • BS/BA-BSN (18-month Fast Track/2nd Degree, Morgantown Campus): US30500600
      • BSN (4-year, Beckley Campus): US30510000

*Students should read the criminal background check information very carefully!

    • West Virginia has recently joined the Multi-state Licensure Compact.  Please note, to apply for the multi-state RN license, West Virginia must be your primary state of residence.  Your primary residence would need to remain in West Virginia.  If you are not a West Virginia resident, you can apply and be considered for a single state WV RN license.

Background Check Information

For West Virginia State Board Applicants, state background checks MUST be completed in West Virginia AND in the state of the student's legal residence.  Please click here for instructions.

For WV, you can schedule your appointment for fingerprints at:  https://uenroll.identogo.com/  Please be sure to input the required Service Code: 228NJX

    • For those applying to a state other than West Virginia, please check your state’s Board of Nursing website for application and background check requirements and instructions. Use the following link to Find your State Board of Nursing.

Register for the NCLEX at Pearson Vue

  • Students must register for the NCLEX online at Pearson Vue through the following website: http://pearsonvue.com/nclex/
  • You will need to pay a $200 fee to Pearson Vue
  • When you set up your account, please input the appropriate school program code: BSN:  US30508000   or   BSBA-BSN:  US30500600 or BSN at Beckley: US30510000

Transcripts

Students can request their transcript through their MIX account. Students should check their unofficial transcript after May 17, 2020, and only order their transcript after their degree is awarded, not pending. The Office of Student Services will send an email when your degree has been posted to your transcript.

The Student is responsible for any outstanding WVU Fees (all fees must be paid or a transcript will not be sent).

NCLEX Exam Self-Reporting

Once students take their NCLEX exam, they must self-report their results using the online form
