Transitional Care course set for Aug. 1-26

The West Virginia University School of Nursing will offer Transitional Care for Faith Community Nurses, an all online course that is designed to provide faith community nurses with background knowledge on the needs of community members who are in the process of transitioning in care.

The course, scheduled for Aug. 1-26, includes information about transitional care from the hospital to home, from the hospital to the nursing home, and from the hospital to home with hospice care.

“This course is helpful for any community-based nurse who is working with patients during times of transition between care settings,” said Angel Smothers, clinical associate professor.

The course uses the national Transitional Care Training Program through the Westberg Institute. The cost of the course is $100, and 15 contact hours are being provided by the WVU School of Nursing.
