Welcome to the Inter-Professional Collaboration for Vulnerable Acute Care Patients: An Academic-Service Partnership
The Inter-professional Collaboration for Vulnerable Acute Care Patients is a Health Resources and Service Administration (HRSA) funded grant project dedicated to improving the health outcomes of vulnerable acute care patient with a focus on cardiovascular surgical and older adult medical patients.
Our research team will incorporate use of the Inter-professional Collaboration for Vulnerable Acute Care Patients Model (ICVAP) to provide team based, patient centered care with the goal of reducing patient length of stay, 30-day hospital readmission rates, and pain. This goal will be achieved by:
- Improving access to safe, effective, and patient-centered care for vulnerable acute care patients by implementing an academic-service partnership
- Increasing the number of diverse nurses, nurse leaders, physicians, pharmacists, other health professionals, and students practicing inter-professional collaboration practices
- Strengthening the capacity for sustained and replicable inter-professional collaboration practices by engaging prepared nurse leaders in IPCP team building opportunities
