The School of Nursing's December Convocation Ceremony for the Graduating Class of 2012 will take place Sunday, December 9, 2012 at ten-o'clock in the morning at the Okey Patteson Auditorium, Health Science Center North, Morgantown, West Virginia. The venue is handicap accessible. Please contact the WVU Office of Disability Services at 304-293-6700 to make any disability-related accommodations for the convocation ceremony.

A reception will be held following the ceremony at the HSC Cafeteria.

All students, please complete the online RSVP form by November 1, 2012.

To order your regalia, please email Chuck Bowers at the HSC Bookstore at sm586@bncollege.com by no later than Friday, October 26, 2012.  Include your name, degree and height so the proper regalia can be ordered.  ALL students must have WVU navy blue regalia in order to participate in the ceremony.

APPLYING FOR GRADUATION AND OTHER IMPORTANT FORMS

Undergraduate Students

   BSN and BS/BA to BSN Students:

Request your transcript through your MIX account. Please note that the degree conferring date is December 28, 2012. Check your unofficial transcript after December 28th and only order your transcript after your degree is awarded, not pending.

All materials below must be turned in to Lorie Stowers, WVU School of Nursing, Office of Student & Alumni Affairs, 6400 HSS, PO Box 9600, Morgantown, WV  26506-9600 no later than October 10, 2012



Graduate Students

MSN Students must submit the Application for Graduation and Diploma by October 1, 2012

PhD and DNP students must apply for graduation through their MIX account. The instructions are below:
1. Log into your STAR account: http://star.wvu.edu
2. Click on the "Student Services and Housing" link
3. Click on "Student Records"
4. Click on "Apply to Graduate for Doctoral Students"

PhD Students:

You will need to provide the “date of qualifying examination” and the “term of admission to candidacy”, as well as your committee members. This has often caused problems for some students. If a committee member isn’t listed you will need to choose a pretend or fake committee member (doesn’t matter who) from the list. Your application will be changed, to reflect the correct committee, when your application is approved by the HSC Graduate Programs Office.

DNP Students:

You will need to provide the “date of qualifying examination” (use your capstone approval date) and the “term of admission to candidacy” (use the current semester: 201208), as well as your committee members. This has often caused problems for some students. If a committee member isn’t listed you will need to choose a pretend or fake committee member (doesn’t matter who) from the list. Your application will be changed, to reflect the correct committee, when your application is approved by the HSC Graduate Programs Office.

ETD Submission:
Doctoral students must also submit their dissertation through the WVU Libraries website by November 30, 2012:

Faculty

Faculty must wear academic regalia. If you do not own your regalia, you may rent caps, gowns and tassels at the HSC or Main bookstores. Please contact Chuck Bowers at sm586@bncollege.com or call 304-293-4849.
